Payment Options

Our office accepts payments by cash, checks, and Visa, MasterCard, American Express, and Discover credit cards as well as CareCredit.

Insurance Policy

When you first visit our office, it’s very important that you bring your current insurance card with you. Whenever possible we ask that you provide us with a copy of your insurance card prior to your appointment to expedite the process. If you are unable to scan or take a picture of your card, front and back, you may e-mail the following pertinent information to us at insurance@doctorfelton.com: insurance company name, member ID, your full name, including your date of birth, as well as the provider phone number on the back of the card, so that we may verify coverage and benefits on your behalf. If you change insurance companies or join another employer’s plan, please be sure to let our insurance department know. If an insurance referral is necessary, please bring the completed form with you to your appointment.

Also, for all new patient appointments, our office charges a one-time New Patient Administrative Fee of $85. This fee is not applied to your insurance, but is in addition to what your copay would be. Again, this fee is only a one-time New Patient Fee.

We will submit all insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within 45 days. If we are not contracted under your insurance, you will be given a copy of services and charges to file with your insurance claim form.

  • HUMANA-PFFS (PHYSICIAN FEE FOR SERVICE ONLY)
  • HUMANA-POS

**WE ARE NOT ACCEPTING ANY NEW MEDICARE OR MEDICARE REPLACEMENT PLANS AT THIS TIME.**

Patients with FMLA, Social Security Disability and Durable Medical Device Requests

Because of the large amount of requests for additional paperwork that the physician would need to fill out, we request that you contact these institutions and have them send their paperwork to you so that you may bring it in to our office on your next visit. The physician will be able to fill this out at the time of your scheduled appointment. There is a $25 fee to fill out any paperwork.
